We are equipped to receive referrals for young people aged up to 17 years and 8 months. Eligibility for our specialist intervention typically extends to those who are already registered on the dynamic support register (DSR) – a crucial mechanism for identifying individuals with significant needs – and who are also engaged with Child and Adolescent Mental Health Services (CAMHS) or dedicated CAMHS learning disability provisions. This structured referral pathway ensures that our support is directed towards those who will benefit most from our focused expertise.
Our core mission is to provide comprehensive aid to both the young person and their family unit when home life becomes exceptionally challenging, and the strain feels as though it's nearing a critical juncture. We understand that families can reach a 'breaking point' where additional, tailored support becomes not just beneficial but absolutely essential. It is at such junctures that our team steps in, offering a lifeline of professional guidance and practical strategies. The mechanism for accessing our services is streamlined through the established dynamic support registers that operate across the various boroughs within the Pennine Care NHS Foundation Trust’s extensive service area. This encompasses regions such as Bury, Heywood, Middleton and Rochdale, Oldham, Stockport, Tameside, and Glossop, ensuring that local populations can readily access the support they require.
Operating on a model akin to the 'Ealing Model of Intensive Support', our team actively works within the community, striving to support young people in their familiar environments. We foster strong collaborative partnerships, including a significant alliance with Barnardo's keyworkers, to ensure that families are seamlessly connected with local support networks and access to valuable short break services, which can provide much-needed respite. Furthermore, we extend our expertise through invaluable consultation services for professionals working directly with young people and their families.
