For individuals aged 65 and over experiencing significant and persistent mental health challenges, as well as those under 65 with early-onset dementia, the Older Adult Community Mental Health Team (OA CMHT) on the Fylde Coast offers dedicated support. Our office can be reached at 01253 957 350.
Our comprehensive services encompass a range of vital provisions, including thorough assessment, accurate diagnosis, effective treatment strategies, and valuable educational resources. A core aspect of our approach is the development of highly personalised care plans, meticulously tailored to meet each individual's unique needs and circumstances.
Accessing our services typically begins with a referral from a General Practitioner (GP). This initial step is crucial as it ensures that any underlying physical health concerns are either fully addressed or comprehensively ruled out before mental health support is initiated. Once a referral is made, it is then reviewed by our Single Point of Access (SPoA) team. This team plays a pivotal role in triaging and processing all incoming referrals. Their expertise ensures that each referral is appropriate for our service and that it is subsequently allocated to the most suitable and relevant team for further action. This streamlined process helps to guarantee that individuals receive the right support from the right professionals in a timely manner.
Our commitment extends to fostering well-being and independence within the community. We understand that mental health needs can be complex and deeply personal, which is why our team comprises experienced mental health professionals dedicated to providing compassionate and effective care. We aim to empower individuals to manage their conditions, improve their quality of life, and maintain their connections within the community. This holistic approach underpins our philosophy, ensuring that care is not just about managing symptoms, but about supporting the individual in their entirety.
