The Older Adult Community Mental Health Team (OA CMHT) in Central Lancashire offers dedicated support to individuals aged 65 and over who are experiencing significant and ongoing mental health challenges. This service also extends to those under 65 with a diagnosis of early-onset dementia.
Our team provides comprehensive services, including thorough assessments to understand individual needs, diagnosis of mental health conditions, and the delivery of tailored treatment plans. We also focus on education to empower both patients and their families, and we collaborate to create highly individualised care plans that address specific requirements.
To ensure appropriate care, referrals typically originate from GPs. This initial step is crucial for addressing or ruling out any underlying physical health concerns before mental health support begins. These referrals are then reviewed by the Single Point of Access (SPoA). The SPoA team is responsible for triaging and processing all incoming referrals, ensuring they are suitable for our service and directed to the most appropriate team for support.
Our team is multidisciplinary, comprising various professionals such as community mental health nurses, occupational therapists, psychiatrists, and psychologists. We aim to provide a seamless journey for our service users, focusing on promoting recovery and maximising their potential. We offer support and treatment for complex dementia presentations and intricate functional mental illnesses, particularly when primary care interventions haven't fully resolved the issues. Our services also include care coordination under the Care Programme Approach and the initiation and monitoring of mental health medication.
