The fundamental principle underpinning our approach is the profound belief that recovery is often more effective, and individuals generally feel safer and more at ease, when supported within the familiar comforts of their own residences. Even during periods of significant distress, our comprehensive support network is meticulously designed to empower people to maintain their independence and continue leading as normal a life as circumstances permit. This home-centric model prioritises stability and continuity, aiming to avert unnecessary hospital admissions wherever clinically appropriate.
The core objective of the CRHT team is to collaborate closely with patients to facilitate their remaining outside a hospital environment. In scenarios where a brief period of inpatient care becomes absolutely essential for safety or intensive treatment, our teams are proficient in coordinating such admissions. Crucially, they simultaneously develop a robust and personalised discharge plan to ensure a smooth and swift transition back to the individual's home as soon as their condition stabilises. Once an individual's immediate crisis has de-escalated and the intensive support of the CRHT is no longer required, but ongoing care remains necessary, we ensure a seamless handover. This involves referring them back to their local Community Mental Health Team, guaranteeing continuous and appropriate care pathways. Our multidisciplinary teams comprise a diverse group of highly skilled professionals, including experienced psychiatrists, clinical psychologists, specialist nurses, occupational therapists, and dedicated support workers. This comprehensive skill set enables us to provide holistic and individualised care tailored to each person's unique needs during a crisis.
Accessing the support of our Crisis Resolution Home Treatment team typically begins with a referral. If you believe you require this level of intervention, your initial step should be to consult your General Practitioner (GP) to discuss your situation and request a referral. Beyond GP referrals, we also welcome submissions from a variety of other crucial sources, ensuring broad accessibility to our services. This includes other healthcare professionals such as midwives, health visitors, and practice nurses, who may identify individuals in need of crisis intervention. Emergency services and first responders, often on the front lines of mental health crises, can make direct referrals to our teams.
