This service is dedicated to offering crucial, intensive assistance to individuals experiencing an acute mental health crisis, right in the comfort of their own homes. Our aim is to prevent the need for admission to a mental health inpatient ward, allowing people to remain in their familiar surroundings during a challenging period.

A key component of our provision is the Rapid Response service, which operates round the clock, 365 days a year, to address both emergency and urgent referrals. For immediate or pressing concerns, referrals should always be directed through our Single Point of Access, which is similarly available 24/7. To ensure continuous support and peace of mind, every individual receiving our care is furnished with a crisis card detailing essential contact information for emergencies.

Our team is equipped to support adults over the age of 18 who are residents of Kensington & Chelsea or are registered with a local GP in the area. We primarily assist those undergoing an acute phase of serious mental illness, or where there's a strong suspicion of such a condition. This encompasses individuals with a dual diagnosis, such as those also managing a learning disability or issues with substance misuse. Furthermore, our services extend to people experiencing a relapse of a pre-existing long-term mental illness. We are particularly focused on safeguarding individuals who are vulnerable or pose a risk of harm to themselves or others, and who cannot be safely managed in a less intensive community setting.

Referrals to our service are accepted from a range of other specialist mental health provisions. These include the Single Point of Access, Community Mental Health Teams, Early Intervention Teams, and Assertive Outreach Teams. We also work closely with Psychiatric Liaison Services and Acute Inpatient Wards, often facilitating an earlier and smoother discharge for patients. Any other secondary specialist teams can also make referrals, as can out-of-area hospitals when a service user under the care of CNWL has been assessed as requiring home treatment.
